Decompressive Craniotomy after Traumatic Brain Injury: Post Operative Clinical Outcome
نویسندگان
چکیده
Background: A decompressive craniotomy can relieve critically increased intracranial pressure. Objective: to assess the postoperative clinical outcome and compare it with other series. Methods: (a) patient selection: patients presented to the Neuroemergency unit in Kasr El-Eini Hospitals from January 2006 to December 2007 with traumatic brain injury and fulfilling the following criteria 1) clinical deterioration to Glasgow coma scale (GCS) 11 or presented to us with GCS (5-11) and refractory to conservative treatment of increased intracranial tension, 2) age ranging from 20 to 60 years, 3) radiological criteria: unilateral brain swelling with midline shift. (b) procedure: decompressive hinge craniotomy. (c) Evaluation of postoperative clinical outcome by the Glasgow outcome scale (GOS) at the time of hospital discharge and at 6-months thereafter, then comparing the clinical outcomes with those of in other researches. Results: The overall outcomes at 6months follow up were good in 9 patients (45%), fair in 2 patients (10%), poor in 9 patients (45%).mortality in 6 cases (30%), vegetative state was seen in 3 cases (15%), overall survival was 70% with favorable outcome in 65% of the survivors. Conclusion: Age and initial posttraumatic GCS remain to be the most important factors in determining the postoperative clinical outcome. Decompressive hinge craniotomy provided favorable clinical results in nearly 45% of patients who were otherwise most likely to die. (Egypt J Neurol Psychiat Neurosurg. 2010; 47(2): 255-259)

Prospective randomized evaluation of therapeutic decompressive craniectomy in severe traumatic brain injury with mass lesions (PRECIS): study protocol for a controlled trial
BACKGROUND For cases of severe traumatic brain injury, during primary operation, neurosurgeons usually face a dilemma of whether or not to remove the bone flap after mass lesion evacuation. Decompressive craniectomy, which involves expansion of fixed cranial cavity, is used to treat intra-operative brain swelling and post-operative malignant intracranial hypertension. However, due to indefinite...
متن کاملEffects of unilateral decompressive craniectomy on patients with unilateral acute post-traumatic brain swelling after severe traumatic brain injury
INTRODUCTION Acute post-traumatic brain swelling (BS) is one of the pathological forms that need emergent treatment following traumatic brain injury. There is controversy about the effects of craniotomy on acute post-traumatic BS. The aim of the present clinical study was to assess the efficacy of unilateral decompressive craniectomy (DC) or unilateral routine temporoparietal craniectomy on pat...
متن کاملPrimary decompressive craniectomy is associated with worse neurological outcome in patients with traumatic brain injury requiring acute surgery
BACKGROUND The role of decompressive craniectomy in treating raised intracranial pressure (ICP) after traumatic brain injuries (TBI) is controversial. The aim of this study was to assess the differences in prognosis of patients initially treated by decompressive craniectomy, craniotomy, or conservatively. METHODS We conducted a single-center retrospective study on adult blunt TBI patients adm...
متن کاملDecompressive Craniectomy in Traumatic Brain Injury:Factors Influencing Prognosis and Outcome
Background and Aim: Decompressive craniectomy can be life-saving for patients with severe traumatic brain injury, but many questions about its ideal application, indications, timing, technique, and even the definition of success of decompressive craniectomy remains unclear. The aim of this study was to assess the factors associated with prognosis and outcome of patients with traumatic brain inj...
متن کاملPrognostic predictors of outcome in an operative series in traumatic brain injury patients.
BACKGROUND Although several prognostic factors for traumatic brain injury (TBI) have been evaluated, a useful predictive scoring model for outcome has yet to be developed for TBI patients. The aim of this study was to determine independent predictors and develop a multivariate logistic regression equation to determine prognosis in TBI patients. METHODS A total of 13 different variables were e...
